One narrative, four diseases, and they interact

The format rule is doing real work here, so read it before the questions. You must write one narrative with an introduction and a conclusion rather than a numbered set of answers, and that constraint exists because M.K.'s conditions are not four separate problems. Her chronic bronchitis, her right-sided heart failure, her hypertension and her type II diabetes are causally linked, and a numbered answer sheet lets you describe each in isolation without ever making the connection. Chronic hypoxaemia from bronchitis produces pulmonary vasoconstriction; sustained pulmonary hypertension loads the right ventricle; the right ventricle fails, which is what her distended neck veins and peripheral oedema are telling you. That chain is cor pulmonale, it explains three of her findings at once, and it is the single most valuable paragraph you can write. Write that chain out before you answer any individual question, because every later answer refers back to it.

Answer every question, but let the narrative carry them. Name her clinical findings and say which mechanism produces each — the morning cough with sputum, the light-headedness, the distended neck veins, the oedema. Use the actual hypertension staging thresholds rather than describing her pressure as high, and connect her medications to the mechanism they act on rather than listing their classes. For the lipid panel, say what the specific values put her at risk of and why diabetes multiplies that risk. Interpret the HbA1c against the diagnostic and target thresholds and explain what glycated haemoglobin actually measures, since that is the part most answers skip. Three sources minimum, one of them not from the internet, and the textbook does not count — so plan a library trip or a print reference before you start writing, because that requirement catches people at the very end. Name the non-internet source in your reference list so a marker can see the requirement was met.

Her current symptoms include chronic cough, more severe in the mornings with sputum, light-headedness, distended neck veins, excessive peripheral edema, and increase urination at night. Her current medications include Lotensin and Lasix for the hypertension along with Glucophage for the Type II diabetes mellitus.
