Two additions to assessment, and a role to answer from

Two things are being added to a standard comprehensive assessment and they add different kinds of information. Environmental history means where the patient lives and works and what that exposes them to: occupational exposures, housing quality, water and air, neighbourhood food access, and the social conditions that determine which of those a patient can change. It is routinely omitted because it takes time and rarely changes today's prescription, which is exactly why it belongs in an argument about comprehensiveness. Genomic information means family history at minimum, and increasingly variant results the patient may already hold from clinical or direct-to-consumer testing. Both make the assessment more predictive of what will happen rather than more descriptive of what is happening now. Then answer through your role, because the prompt asks you to and the two roles genuinely diverge. As an advanced practice nurse the argument is at the encounter: which questions get added, how you ask about occupation without turning it into small talk, what you do with a family history that suggests referral, and how you respond when a patient brings a direct-to-consumer result whose clinical meaning is limited. As a nurse executive the argument is structural: whether the assessment template has fields for exposure at all, whether staff have the genomic literacy to act on what they collect, whether referral pathways to genetic counselling exist, and what protecting an extra few minutes per assessment costs across a service. Naming which role you are answering from in the first sentence is the single easiest way to make the post specific. That one sentence does more for the post than a paragraph of advocacy.
