One method, and evidence-based data is the constraint

The phrase that constrains this post is includes using evidence-based data. An innovative idea is not enough — the method has to be one where data drives the intervention, which rules out most of what first comes to mind. Methods that do qualify include risk stratification using registry or claims data to identify older adults at high risk of falls or admission and target an intervention at them; remote monitoring where a threshold in the data triggers a clinical response; deprescribing programmes driven by explicit criteria applied to a medication list; and comprehensive geriatric assessment used to allocate services by measured need rather than by referral order. In each of those, the data is not evidence supporting the idea, it is the input the method runs on. The innovation is in how the data decides who gets what, not in the intervention itself, and that distinction is exactly what the wording is testing.

Both follow-up questions have to be answered, and the second is where posts get vague. Impact on care means what changes in practice: who gets seen, in what order, by whom, and what stops happening to make room for it. Anticipated outcomes should be stated as things you could put a number on — falls per thousand patient-days, thirty-day readmissions, potentially inappropriate medications per patient, days at home in the last year of life. Naming the measure also forces honesty about the timescale, since most of these move slowly. Two further points strengthen the post considerably: say where the data comes from, because a method requiring data your setting does not collect is not implementable, and acknowledge that risk stratification inherits whatever inequity is in the historical data, which in an older population usually means undercounting people who have had less contact with services. Say so plainly.

Hello class and welcome to the class and I will be your instructor for this course. This is a -week course and requires a lot of time commitment, organization, and a high level of dedication. Please use the class syllabus to guide you through all the assignments required for the course.
