Four questions, and the last one runs both ways
Get the level of theory right first, because the prompt specifies middle-range and the distinction is the reason it does. A grand theory offers a philosophy of nursing and almost no operational guidance, which is why it cannot found a practice improvement project. A practice theory is so situation-specific it does not travel. Middle-range theories sit between: they address a limited aspect of practice, contain concepts that can be measured, and generate testable propositions. Unpleasant Symptoms, Self-Efficacy, Comfort, Health Promotion and Transitions are all defensible choices. Compare on real axes rather than describing each in turn — the phenomenon addressed, the core concepts and whether instruments exist for them, the propositions each makes, and the point of practice each implies as a lever. Say which phenomenon each theory addresses in one line before comparing them, so a reader can see that the pair is genuinely different rather than two names from the same chapter.
Then apply one as a foundation for direct practice improvement, and foundation means it does structural work rather than appearing in the introduction. A theory earns that role by supplying three things: an explanation of why the intervention should change the outcome, guidance about what to measure, and a prediction that could be wrong. Self-efficacy theory applied to a self-management project supplies all three — it says confidence in performing a behaviour predicts performance, so the intervention targets mastery experiences and verbal persuasion, the measure includes a self-efficacy scale as well as the clinical outcome, and it predicts that any improvement should be preceded by a rise in efficacy scores. Finally, the last clause runs in both directions: theory guides practice by telling you what to attend to and what to expect, and practice and research guide theory by producing findings that force a concept to be revised. Say what your project would contribute back if the prediction failed.
