Five theories are named — pick one and use it

The reading offers several theories and they are not interchangeable, so the post should commit to one and let it do real work. Accident causation models treat harm as the alignment of latent weaknesses through layers of defence, which directs you toward system barriers rather than individual vigilance. Human error frameworks separate slips and lapses from mistakes and violations, and the distinction matters because the remedies differ — forcing functions for slips, training and decision support for mistakes, and a look at the conditions producing them for routine violations. Resilience engineering inverts the whole question and asks why things usually go right, which points at adaptive capacity rather than at defect counting. Choosing one and following it through is what produces measures that hang together instead of a shopping list. System migration deserves a mention as the odd one out, since it describes an organisation drifting steadily toward the boundary of safe operation without any single decision to do so.

Then honour the word proven. A measure is proven when there is evidence it changed an outcome, not when it is widely adopted or sounds obviously right — plenty of safety interventions are both and have thin evidence behind them. So cite the evaluation, not the guideline. Be careful too about what your measure measures: process measures like checklist-completion rate are easy to collect and easy to game, while outcome measures like infection rate are what actually matters and are noisier at unit level. The strongest posts propose both together and say why, because a process measure that moves while the outcome does not is exactly the finding a safety programme needs to detect early. Tie the choice back to your theory: the measures should be the ones your chosen framework predicts will shift. Say how often the measure would be collected, since one reported quarterly cannot detect a decline in time to act on it.
