A realist review reports mechanisms, not effect sizes

The methodology matters here more than it usually does. A realist review does not pool effect sizes across studies to produce an average answer about whether something works. It assembles context-mechanism-outcome configurations: a statement that in these circumstances, this mechanism produces this outcome. So when the prompt asks you to discuss one of the outcomes identified for effective transformation, the outcome on its own is only a third of the finding. Taking, say, distributed leadership and reporting that it supports transformation drops both the mechanism that makes it work and the conditions under which it does — which is precisely the content a realist review exists to supply and precisely what most posts leave out. Realist synthesis exists because trials of complex interventions kept contradicting each other, and the contradictions dissolve once context is part of the finding rather than noise around it. That is why the article rewards being read rather than summarised.

So structure your post around the full configuration. Name the outcome, state the mechanism the review proposes, and give the context in which it operates. Then run the implications for your own practice, which is the second half of the prompt, and run them honestly: the useful question is whether the conditions the mechanism depends on exist where you work. Large-system transformation findings are drawn overwhelmingly from whole-system reform programmes with multi-year horizons and executive sponsorship, and a DPI project on one unit over eight weeks does not have those conditions. That is not a reason to discard the finding — it is a reason to say which part of the mechanism you can reproduce at your scale and which you cannot. A post that names the mismatch is stronger than one that claims a system-level insight applies unchanged to a ward. Say which element you would carry across and how it would be recognisable at your scale.
