One example, from your practice, with an outcome attached
This is an experience question with a hard requirement buried in it: the collaboration must have enhanced patient outcomes, so the example needs an outcome. That is where most responses fall down. A story about a well-run huddle, or a pharmacist and a nurse communicating promptly, describes good process and stops there. What is being asked for is the link: what changed for the patient because the collaboration happened. Choose an example where you can say something concrete — a deterioration caught earlier, a medication error intercepted before administration, a discharge that did not bounce back, a goals-of-care conversation that stopped a futile transfer. Choose an example with a countable outcome rather than an averted disaster, since a claim about what would otherwise have happened is hard to weigh. It also helps to pick a case where the disciplines genuinely needed each other, rather than one where a single clinician would have reached the same conclusion given time.
Then make the mechanism visible rather than attributing the result to collaboration as such. Collaboration is not itself a mechanism; it is a category. The mechanism is usually something specific — information that one discipline held and another needed, an authority gradient that was flattened enough for a concern to be raised, a decision made once with everyone present instead of three times sequentially. Naming which of those operated is what makes the example analytic. It is also worth grounding the anecdote in the wider evidence, which is more nuanced than the enthusiasm around teamwork suggests: structured communication tools and team training have reasonable evidence behind them, while collaboration in general has been hard to link to outcomes precisely because it is so broadly defined. Saying that shows you can hold your own experience and the literature in view at once. Say who else had to be present, since that identifies what the collaboration depended on.
