Which of the six systems should you choose?

Six systems are offered and they are not interchangeable, so the choice you make in the first paragraph decides how easy the next nine pages will be. MYCIN and AAPHelp are historical research systems with published descriptions of their reasoning — MYCIN's backward-chaining production rules with certainty factors, AAPHelp's Bayesian approach to acute abdominal pain — which makes the design and methods of reasoning sections straightforward and the interoperability and internet sections a stretch, because neither system was built for a networked world. ISABEL and DXplain are the opposite: living products with real integration stories and thin public detail about their internals. GermWatcher, a surveillance system for nosocomial infection, sits between the two and has the clearest data-integrity story of the six. Choose according to which sections you would rather write well, then say in your thesis why that particular system is worth evaluating at all.

Methods of reasoning is a technical term and the marker is checking whether you know it. Rule-based inference, Bayesian probability, decision-analytic modelling with utilities, pattern matching, neural or other machine-learned classification are different mechanisms, and your system uses one or two of them rather than all. Clinical intent and mechanisms of intervention are also distinct: intent is the clinical problem the system was designed to solve, while mechanism of intervention is how the advice actually reaches the clinician — an interruptive alert, a ranked differential returned on request, a report generated overnight, a suggested order set. A paper that says the system helps doctors make decisions has skipped both. Where the historical record is silent on a section, say so explicitly and reason about what the system would require today; that is a legitimate evaluation, and it reads as judgement rather than as a gap. Inventing plausible detail to fill the section is the one thing that cannot be recovered.

The last two requirements are the ones that separate a passing paper from a strong one. Ten data elements with permissible values means a real data dictionary fragment, not ten field names: patient age as an integer 0 to 120, temperature in Celsius to one decimal, specimen source constrained to a coded list, culture result constrained to positive, negative or pending. Give the element, the type, the constraint and where the value comes from, because provenance is what the data integrity section is really asking about. The compliance policy then has to be a policy — a scope, who it binds, the standards it enforces such as HL7 or ASTM messaging, UMLS-based terminology, the review cycle, and what happens when a system fails to comply. Write it as a document a reader could adopt tomorrow rather than as a paragraph of recommendations, and reaffirm the thesis in the conclusion, since the assignment asks for that explicitly.

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes. Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages. Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor. The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.
