Senge's five disciplines are the test, not the vocabulary
This prompt is built around a counterfactual, and counterfactuals are only persuasive when the mechanism is specific. So choose the situation for its diagnosability rather than its drama: a repeated near miss nobody escalated, a new documentation system nobody used as intended, a departmental fix that quietly made another department's workload worse. Then name which of the learning-organisation disciplines was absent, because Senge's framework gives you five and they fail differently. Personal mastery is about individual capability, mental models about the assumptions people do not know they hold, shared vision about a goal that is genuinely common rather than announced, team learning about how a group thinks together, and systems thinking about seeing the whole rather than a part. A situation where a fix in one unit created a problem in another is a systems thinking failure; a situation where everyone privately saw the risk and nobody raised it is a team learning and mental models failure.
Once the missing discipline is named, the counterfactual becomes concrete. What would have been different is not simply that the outcome would have improved. It is that a specific practice would have existed — an after-action review that asked why rather than who, a forum where a frontline observation reached the person who could act on it, a map of the process that showed the downstream unit. Say who would have done what, at which point, and what they would have seen. Then handle the research requirement honestly. Current research means recent empirical work on organisational learning in health care, not a citation of the 1990 book alone. Look for studies of psychological safety, of incident reporting that changed practice, or of change models applied in health systems, and use them to argue that the practice you described actually produces the effect you are claiming for it.
