It is one of the classic causes of drug-induced lupus, so the case invites you to consider a transient lupus-like syndrome — and then tells you the medication was changed years ago with no change in symptoms, which argues against it. Use that. The pathophysiological differentiation the prompt asks for runs as follows: chronic SLE is a systemic autoimmune disease in which loss of tolerance to nuclear antigens produces autoantibodies, immune complexes deposit in tissue, complement activates and multi-organ inflammation follows, with a relapsing course and genuine renal, neurological and haematological risk. Drug-induced lupus is a reaction to a specific agent — hydralazine, procainamide and others — in which the offending drug alters antigen presentation; it produces antihistone antibodies, spares the kidneys and central nervous system in most cases, and resolves within weeks to months of stopping the drug.

That difference drives the laboratory answer. Antinuclear antibody is sensitive but not specific and is positive in both; anti-double-stranded DNA and anti-Smith are the specific markers for SLE, and anti-dsDNA titres track disease activity and nephritis. Antihistone antibodies point toward the drug-induced form, and low complement C3 and C4 point toward active SLE. Add the tests that stage organ involvement rather than confirm the label: complete blood count for cytopenias, creatinine, urinalysis for proteinuria and casts, and antiphospholipid antibodies. Fibromyalgia then separates on evidence rather than on symptoms: it produces widespread pain, fatigue and sleep disturbance with no rash, no inflammatory markers, no autoantibodies, no organ damage and a normal examination for synovitis — and the two conditions coexist often enough that the real clinical error to guard against is assuming that one positive ANA result explains every symptom a patient in front of you happens to have.
