Four drugs, two syndromes, and a state-specific question

Notice that the four drugs are not four of the same thing. Amitriptyline is a tricyclic, bupropion is a norepinephrine-dopamine reuptake inhibitor, and fluoxetine and paroxetine are both SSRIs that nonetheless differ in ways that matter clinically. That spread is deliberate, so let the side effects follow from mechanism: amitriptyline's anticholinergic and antihistaminergic activity produces dry mouth, constipation, sedation and orthostatic hypotension, and its cardiac toxicity in overdose is the reason it is rarely first line; bupropion lowers the seizure threshold and is activating, which makes it the wrong choice in eating disorders and a useful one where sexual side effects or fatigue are the problem; fluoxetine's long half-life makes discontinuation easier but interactions longer-lasting; paroxetine's short half-life and anticholinergic activity make discontinuation symptoms markedly worse. Pair each with a management strategy, which is what the prompt asks for. Say which of the four you would reach for first in an uncomplicated case.

The two syndromes are the safety core and the distinction is examinable. Serotonin syndrome develops over hours, follows a serotonergic agent or combination, and presents with hyperreflexia, clonus, agitation and diarrhoea; management is withdrawal of the agent, supportive care and cyproheptadine in severe cases. Neuroleptic malignant syndrome develops over days, follows a dopamine antagonist or its withdrawal, and presents with lead-pipe rigidity, hyporeflexia and marked hyperthermia; management is withdrawal, cooling and dantrolene or bromocriptine. Clonus versus rigidity and hours versus days are the discriminators worth memorising. Then answer the specific questions: sertraline is generally preferred in breastfeeding, paroxetine is usually avoided in older adults because of anticholinergic burden and falls risk while SSRIs with fewer interactions are preferred, and the benzodiazepine question needs your own state's rules rather than a general answer. Say where you would look these up in practice as well, because nobody carries this from memory and naming your reference is a professional answer rather than an evasion.

ADDITIONAL INSTRUCTIONS FOR THE CLASS Discussion Questions (DQ) Initial responses to the DQ should address all components of the questions asked, include a minimum of one scholarly source, and be at least 250 words. Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source. One or two sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words. I encourage you to incorporate the readings from the week (as applicable) into your responses. Weekly Participation Your initial responses to the mandatory DQ do not count toward participation and are graded separately. In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies. Participation posts do not require a scholarly source/citation (unless you cite someone else’s work). Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week. APA Format and Writing Quality Familiarize yourself with APA format and practice using it correctly. It is used for most writing assignments for your degree. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content. It is best to paraphrase content and cite your source.
