Which columns of the organizer actually carry marks?
This looks like a data-entry exercise and is graded like a judgement exercise. Five of the seven columns — data source, primary content, population targeted, demographic data, schedule — can be filled by reading each source's own documentation carefully. The other two cannot. Is this a source of primary or secondary data? requires you to decide what the data was originally collected for, and how, when and where the information might be used requires you to say something the source's own website does not. Those two columns are where an organizer stops being a transcription and starts being an argument, so budget your effort accordingly rather than spreading it evenly across a grid that looks uniform. The grid's uniform appearance is what misleads: seven columns of equal width imply seven tasks of equal weight, and they are not. Read the two judgement columns first and let them drive which five sources you pick.
The primary-versus-secondary distinction is the single thing most often got wrong, because people apply the wrong test. The question is not who published the data or whether you personally collected it. It is whether the data was generated for the purpose it is now serving. A national survey that interviews households to measure health care use is collecting primary data, even though you are a secondary user of it. An administrative database built from hospital billing records is secondary data no matter how authoritative the agency releasing it, because those records exist to get claims paid and were repurposed for research afterwards. Apply that test source by source and the column becomes defensible instead of guessed. Write the reason into the cell rather than only the verdict, because a bare "secondary" is indistinguishable from a guess and the reasoning is the part being assessed.
One practical choice shapes the whole submission: pick five sources that differ from each other. The report draws on more than 250 measures across many settings, so five household surveys would fill the grid while demonstrating nothing about range. A survey, an administrative claims database, a registry, a vital-statistics or census source, and a facility-reported measure set will make the schedule and primary/secondary columns actually vary — which is what lets the final column say something useful about when each source can answer a question and when it is already too old. Deliberate variety also protects the final column from repetition: five sources that collect differently get used differently, so each row has something distinct to say about when and where its data can answer a quality question rather than restating the row above it.

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes. Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages. Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor. The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.

Search sequence
- 1.Open the 2017 NHQDR data sources list from the topic materials first and shortlist candidates from it, rather than choosing sources and checking afterwards whether the report used them.
- 2.For each shortlisted source, find its own methodology or technical documentation page — that is where sampling frame, target population and release schedule are stated precisely.

Common mistakes
- Deciding primary versus secondary by who published the data. The test is what the data was originally collected for — an administrative claims database stays secondary however authoritative the agency releasing it.
- Choosing five sources of the same kind, usually five national surveys, so the schedule and primary/secondary columns come out nearly identical and the comparison demonstrates nothing.
- Filling the how/when/where column with a restatement of the primary-content column instead of naming a question the source could answer.
- Reading release frequency as currency. A source published annually can still describe a data year two or three years back, and that lag is what decides whether it can answer a question about present performance.
- Substituting a newer NHQDR edition without saying so. The brief specifies the 2017 report and its data sources list.
- Treating the demographic-data column as a yes/no. Which characteristics are captured, and at what granularity, is the part that matters for a disparities report.
