The prompt sets three constraints on the intervention and they eliminate most first drafts. It must be an independent, specified nursing change intervention; it must not require a provider prescription; and there must be a comparison population not currently receiving it, plus a timeframe for implementing the change. That rules out anything pharmacological and anything requiring a physician's order, which leaves what nursing owns: structured education programmes, culturally tailored dietary counselling, family-inclusive self-management support, community health worker follow-up, motivational interviewing at clinic visits. Specified means you say what it consists of and how often — a six-week culturally adapted group education programme delivered in Spanish and English with a family member present is specified; promoting healthy lifestyles is not, and a reader cannot tell whether it happened. The same specificity applies to the comparison: adolescents receiving usual care is acceptable only if you say what usual care currently consists of in your setting, because otherwise the comparison cannot be measured against anything.
The population is the paper's strongest asset and should not be flattened into adolescents with diabetes. Hispanic American adolescents newly diagnosed with type 2 diabetes are a group with documented higher prevalence, and the evidence base points at specific mechanisms — dietary patterns embedded in family and cultural practice, language access, insurance and clinic access barriers, and the fact that decisions about food in this age group are frequently made by a parent rather than the patient. That last point argues directly for a family-inclusive intervention, and it gives your PICOT a rationale grounded in the population rather than a preference you happen to hold.
