Searching Databases: PICO(T) Question Discussion Guide

A clinical inquiry is not yet a question
The analogy the prompt opens with is more than decoration. You decide what matters before you start looking, because the criteria determine the search rather than the other way round. A clinical inquiry is the vague version — something you have wondered about on the unit. A PICO(T) question is that inquiry forced into components specific enough to be searched: population, intervention, comparison, outcome and, where it applies, time. The step people skip is the comparison, and it is usually the most useful one, because 'compared with usual care' or 'compared with hourly rounding' turns a topic into a question that a database can answer. The outcome has to be measurable too. Better patient satisfaction is a wish; a change in a named score, in fall rate per thousand patient days, or in thirty-day readmission is something a study can report and you can find.
Then post the search itself, because that is the part a marker can assess. Give the database you used and why it fitted — CINAHL for nursing-specific literature, PubMed or PMC for biomedical breadth, Cochrane for synthesised evidence, PsycINFO for behavioural outcomes. Give the terms you actually typed, the Boolean operators, any controlled vocabulary such as MeSH headings, the filters you applied for date and study type, and how many results each step returned. That narrowing sequence is the evidence you searched systematically rather than typed a sentence into a box. Say what you would change next time: too few results usually means the question is over-specified or the terms are too narrow, while thousands of results means a concept is missing. And note where the vocabulary differs between databases, since a term that works in CINAHL may not exist in PubMed's thesaurus at all.

- Describing the search in general terms instead of posting the strategy.
