Care delivery models

Describing the model is the setup. Whether it is sustainable turns almost entirely on who pays for it and whether that payment survives the pilot funding.

Rapidly changing is the operative phrase. A model chosen once for a population that keeps shifting is wrong within two years, so the answer is a process rather than a match.

The audience is administrators, and that is a graded fact rather than a framing device. A clinically excellent case that never mentions cost or risk will not survive the room it is written for.

Choosing the community organisation decides the whole assessment — and the scoring guide wants policy provisions at three levels, not one.

Three tests in one sentence: innovative, interdisciplinary, and demonstrably better for outcomes. Case management passes the second and struggles with the first.

Describe the nurse leader's role in determining the model of care, describe one model with its pros and cons, examine the return on investment for patient quality and outcomes, and explain the human and financial resources implementation would require.

A discussion post describing the patient care delivery method used on your own unit, comparing it with the model from the readings, identifying the modifications that make it different, and arguing whether it is effective there.
