The timeline and the partnership requirements are the two most often skipped, and they are the ones that turn a proposal into something a reader can believe would happen.

A 1,500 word APA paper reviewing the Healthy People 2020 initiative's objectives, goals and indicators, then using your city or county public health department or the CDC to choose three population or community based problems from the topics listed there, discussing how each affects your community, how each was identified, and a plan of action to contain them based on Healthy People 2020, including the zip code of the chosen area and a minimum of four evidence-based references in Arial 12.

A three to five page needs assessment outline describing and documenting the health status issue a proposed project will address and the population it will serve, including baseline prevalence and demographic data with supporting racial and ethnic detail, the prevalence of health indicators in the geographic area, the current availability of preventive services addressing the issue, relevant barriers in the service area, and gaps in service provision, in knowledge, and in provider and agency capacity to screen, assess, intervene and coordinate.

Evaluate community demographic, environmental and epidemiological data to diagnose a widespread population health issue, develop an ethical improvement plan for it, propose criteria for evaluating the plan's outcomes, justify the evidence you relied on, and design a culturally sensitive communication strategy.
